Unable to Start Plex Plugin AGAIN

Last Friday, my Plex turned off and refused to turn back on.
The only change to my setup was a new Volume I added to exclusively hold all media.
Thinking the new volume was the source of my woes, I unplugged it an loaded a previous config, to no avail.

After several restarts of everything I could think of from Jails to the NAS, I installed the plugin again.
The plugin worked fine until yesterday evening when it turned off again. It was reading from the new volume without problems, even worked fine with the copied metadata before it turned off.

I even installed the latest update, which was available when I went to investigate my lack of connectivity.
Tried starting it within the shell and got the following error

Any ideas on what could be causing this? I'd hate to have to keep installing a new instance of Plex every few days.

the "pam_start" error is usually caused when permissions of the jail are changed. this breaks the jail, and a fresh jail needs to be installed. to confirm can you show the output of the following
