Possible HDD failing

I'm getting the following message on my system and I believe it is pointing to a HDD failure:
Aug 28 04:25:40 freenas smartd[2485]: Device: /dev/ada3, 736 Currently unreadable (pending) sectors
Aug 28 04:25:40 freenas smartd[2485]: Device: /dev/ada3, 13 Offline uncorrectable sectors

This is drive 4, is there any way of knowing for sure what is happening?
Also what is the procedure to replace this drive with another?

What's happening, for sure, is that ada3 has over 700 bad sectors. The disk is failing. The manual has click-by-click instructions to replace a failing disk.
