USB Errors?

I just finished getting my network setup, then last night in the middle of my file transfer my server died on me. Now every time I try to boot into freenas I get these errors. When I first rebooted the bios didn't recognize the boot drive, then I moved them to a different port and it saw them. But I'm still getting the same errors.

Specs:
2X xeon 2670
Asus Z9PA-D8
64gb ddr3 ecc ram
norco 4224 chassis
intel X550-T2 NIC
Intel RES2SV240
LSI 9240-8i (flahed to 9211-8i it mode)
6 8TB WD white hdd's
6 10TB WD white hdd's
3 Samsung ssd's
2 samsung flash drives (boot)

Or boot from a small SSD. That's the trend at the moment.

Welp, after some usb switching I found out one of them was causing the issue. If I put them both on onboard usb ports it would boot, but the drive said faulted. I replaced it with a sandisk stick I had lying around for the moment, but I can't get it to mirror.

This is the new error I get:

[EFAULT] 'gpart create -s gpt -f active /dev/da16' failed: gpart: arg0 'da16': Invalid argument

The drive was never used and I selected use use all disk space.

Edit: Looks like the new flash drive was also giving me issues.
