So in the security log that i receive daily in my email and in the error log from my freenas box, i get a lot of repetitive errors.
Jun 2 08:16:48 NAS kernel: (da3:umass-sim3:3:0:0): WRITE(16). CDB: 8a 00 00 00 00 03 5b dc 5d 28 00 00 00 80 00 00
Jun 2 08:16:48 NAS kernel: (da3:umass-sim3:3:0:0): CAM status: CCB request completed with an error
Jun 2 08:16:48 NAS kernel: (da3:umass-sim3:3:0:0): Retrying command
Jun 2 08:16:57 NAS kernel: (da3:umass-sim3:3:0:0): WRITE(16). CDB: 8a 00 00 00 00 03 5c 19 42 68 00 00 00 80 00 00
Jun 2 08:16:57 NAS kernel: (da3:umass-sim3:3:0:0): CAM status: CCB request completed with an error
Jun 2 08:16:57 NAS kernel: (da3:umass-sim3:3:0:0): Retrying command

and it just goes on and on. the only thing that changes is the string of hex with it. i have no idea what this is in reference to or what the issues is caused by. any one have any ideas?

The disk that is device da3 is probably bad/failing. Search around the forums and you'll see a set of commands to determine what disk it is and how to replace it.
