I have noticed that one of my drives has started increasing the error count consistently the last two times it's done a extended offline test. The short offline tests, pass, but the extended ones fail. Looking at the SMART data, it seems like the drive is ok, but I'm wondering if someone can help me understand what is causing the issue so I can try to resolve it, or at least know if I need to start shopping for a replacement disk.

Thank you in advanced.

Hardware:
I have a Freenas-Mini-3.0-E+ running TrueNAS 13.0-U5.1 with 4 WD Red 4TB WD40EFRX. Everything has been running without issue for over 3 years now.

SMART output:
Code:
root@freenas[~]# smartctl -x /dev/ada1
smartctl 7.2 2021-09-14 r5236 [FreeBSD 13.1-RELEASE-p7 amd64] (local build)

It's failing SMART tests, definitely replace it.

Ok, fair point. Is there anything in the SMART data that indicates what exactly is failing? All the SMART attributes seem in good condition.

Indeed, most of the data looks fine, which is more than a little unusual, but not unheard of. My take on this is that SMART tests are typically used as a criterion for in-warranty exchanges, so vendors are naturally encouraged to be loosey-goosey with their pass criteria. If even so the disk fails the test, something must be very wrong.

Code:
Error 1 [0] occurred at disk power-on lifetime: 25346 hours (1056 days + 2 hours)
  When the command that caused the error occurred, the device was active or idle.

  After command completion occurred, registers were:
  ER -- ST COUNT LBA_48 LH LM LL DV DC
  -- -- -- == -- == == == -- -- -- -- --
  04 -- 51 00 00 00 01 01 fb a2 b8 40 00

  Commands leading to the command that caused the error were:
  CR FEATR COUNT LBA_48 LH LM LL DV DC Powered_Up_Time Command/Feature_Name
  -- == -- == -- == == == -- -- -- -- --  ---------------  --------------------
  ea 00 00 00 00 00 00 00 00 00 00 40 08  6d+19:52:51.019  FLUSH CACHE EXT
  61 00 18 00 e0 00 01 88 6e fe b8 40 08  6d+19:52:51.018  WRITE FPDMA QUEUED
  61 00 08 00 d8 00 01 6e 88 15 f0 40 08  6d+19:52:51.018  WRITE FPDMA QUEUED
  61 00 08 00 d0 00 01 6e 44 18 d8 40 08  6d+19:52:51.018  WRITE FPDMA QUEUED
  61 00 10 00 c8 00 01 52 93 f7 f8 40 08  6d+19:52:51.018  WRITE FPDMA QUEUED
That's the error...

Then this line indicates that there's a bad sector found on each extended test in the numbered sector:
Code:
Num  Test_Description    Status                  Remaining  LifeTime(hours)  LBA_of_first_error
# 1  Extended offline    Completed: read failure       40%     26094         4328235704

Seems to be in warranty (assuming 4 years).

What's curious is that it wasn't just remapped and logged... Rather the test just failed outright, multiple times. It must be more than just a bad sector.

I have encountered the same "path", while testing a refurbished WD10EFRX before use: no errors logged, only error count increase and smart test fail everytime (the short one too). Maybe is not so unusual :D
What was courious for me, was the fact the same disk, tested with third party program (Crystal disk info) show an healthy drive... obviously i send it back and get total refund

Yea, I have not warranty path, it seems. 3-year warranty expired Jan. 17, 2024...figures. So I guess I'll look at replacing it when I get a chance. Any idea when TrueNAS will decide it's bad and kick it out? So far the array has been fine, but I'm guessing if this disk gets kicked out the pressure is on to replace it, just trying to gauge how long I have.

No, the array is not fine. It is just that the last of defense has not been breached yet. If you want to wait until then, that is of course your prerogative. But it is risky, because "defense lines" (to continue with the figure of speech) do not break in a linear way. So any second the drive can be gone. What that means for your data is not clear, because you did not specify your pool setup.

If you run TrueNAS this is an indicator that you care about your data. In that case, I would run for a new disk.

Well it seems that the drive was still under warranty for another 20 days, so WD is replacing it under warranty, so they agreed with your assessment, though, surely no justification was needed.

Thank you all for your thoughtful insight in helping resolve this issue. I appreciate it all.
