I am getting the following error message on ada4. Does ada4 refer to sata port number 4?

Absolutely not. If you want to find out which drive it is, at the command prompt type smartctl -a /dev/ada4 and the drive will spit out some data, one piece near the top is the drive serial number. Use this to identify the physical drive. Next, post the output of that command here for analysis, or refer to my link in my signature line to Troubleshoot Hard Drive Guide.
