Unexpected reboot occured during pool scrub

I suspect a hardware failure is in my future. For the second time in 4 months, my system has rebooted during a pool scrub. I'm generally looking for advice on how to pinpoint or predict where my shortcoming could be.

For some background, this system has been issue free and stable for the last 3 years.

Hardware:
Dell r720xd
2x 750w PSU
2x e5-2697v2
192GB DDR3 ECC
12x 8TB HGST NAS
2x LSI 9211-8i (IT MODE)

ESXi 7.0
FreeNAS VM is given 96GB of RAM, and the HBA's are passed through to the VM. FreeNAS has direct access to the disks

Pool Layout
raidz3 - No L2ARC, No SLOG

Pool scrubs occurs on the 1st/15th of every month.
SMART Short tests are scheduled to run weekly
SMART Long tests are scheduled to run every other week
I've scheduled the smart tests and the scrubs so that they don't overlap.

I've attached a copy of the smart tests from all 12 drives.

The following test on one fo the drives, stood out to me, as it did encounter an error. But that was last month. and the drive has since passed additional tests without issue.

I logged into the idrac of my machine, and went through the logs there, and I didn't see anything that stood out. Nor did I see any alerts in the esxi logs. That leads me to beleive one of the drives is beginning to fail, as that's the only hardware that isn't connected to the host.

Are there any other diagnostics or tests I could run that would help me narrow down the issue? So far it seems all is fine, but that clearly isn't the case.

Anything logged in your iDRAC lifecycle log for the event?

Could your HBAs be overheating? Scrub is intensive. Try bumping your base fan speed up in the BIOS (I think it is)?
