I have a pool with 4X WD 4TB Reds. One drive started reporting issues so I went through the process of replacing the failed drive following the official documentation (I dont have the link at the moment). I had spare bays in my chassis so I did not physically remove the drive I just went in pool status using the gui and selected the bad drive and chose replace and went through the steps. During the rebuild process the replacement drive died. I started the process again from the beginning with a another new drive. The resilver process seemed to complete without issues but now the volume still shows as Degraded and when I look a the disk view in Pool Status it looks as follows:

I'm just trying to figure out if this is toast and I need to rebuild this pool from scratch or if it can somehow be salvaged.

Have you physically removed the failed replacement disk?

Have you physically removed the failed replacement disk?
Yes, both the original failed drive and the first attempted replacement drives have been removed. The only drives for this pool that are still present in the chassis are the 3 original drives and the 2nd replacement drive that completed the resilver process. Everytime the server is rebooted the resilver process kicks off again.

Were you able to resolve this?
No, this is my first time coming across this problem and nothing I've googled really helps me as to what I should do next... short of destroying pool and starting over. I've been using FreeNAS for a few years and have replaced a multiple drives both failed and to increase a pools capacity but never had this happen.

Have you found a solution to this? I'm in the same boat right now and haven't found anything saying how to resolve the issue.

Nope, seems nobody knows how to resolve this issue. My plan is to destroy the pool and rebuild it if no solution is presented in the next week or so.
